Records can be provided upon request via CD or flash drive, or sent to you electronically by e-delivery. Please specify your preferred format on the authorization form. Requests are processed within five to 10 days of receipt. For records that are mailed, please allow time for delivery. You or a representative designated by you may pick up the medical records and must show a valid ID at time of pick-up.
