Payer–Provider Collaboration Advances Care to Improve Coordination, Expand Access, and Strengthen Outcomes for New Yorkers
Westchester, N.Y. — June 3, 2026 — MVP Health Care (MVP) and Westchester Medical Center Health Network (WMCHealth) today announced a next‑generation strategic partnership designed to improve care delivery by bringing the health plan and hospital teams together at the point of care. This model enhances support of MVP members during hospitalization and as they transition to recovery.
Through the partnership, MVP’s clinical care managers work alongside Westchester Medical Center (WMC) care teams within the hospital and support members as they transition home and navigate follow‑up care and services. Rather than operating as separate organizations with separate processes, MVP and WMC will function as a connected care team, aligning support in real time across the care experience. By bringing care coordination directly into the clinical setting and extending that support beyond discharge, the partnership helps reduce gaps between hospital and home by better aligning care plans with each member’s clinical needs with their insurance coverage to support stronger outcomes.

According to research published by the National Institutes of Health, gaps in care coordination during and after a hospital stay are a significant driver of avoidable readmissions. Studies show that patients who felt uninvolved in care decisions were 79% more likely to be readmitted, while those who did not understand their discharge instructions were nearly twice as likely to return to the hospital. The partnership is designed to address these challenges through a more integrated approach to care.
The partnership will also expand access to WMCHealth’s nationally recognized advanced and specialty care for MVP members across the Hudson Valley. Through new programs, including expanded pathways to advanced services and virtual specialty consultations, MVP members will have greater access to high‑quality specialty care closer to home and more seamlessly connected to their overall care experience.
